Buffalo, NY. Family Health Center. Educational Success & Urgent Care Coordinator. FT. Benefits. Monday- Friday. $19.00- $20.00/Hour. Family Help Center is seeking a full-time Care Coordinator/Social Worker for a position in our Urgent Access and Educational Success program(s). This position is part of the wraparound service program targeting youth in the Erie County Children's System of Care. Care Coordinators provide home-based Services to assist youth and families with strength-based individualized service planning and provide linkages, coordination, and monitoring of services to improve youth's education experience and promote family stability. Urgent Access Care Coordinator services 6-8 cases, Educational Success Care Coordinator services 8-10 cases enrolled in Erie County Wrap Services. Care Coordinator provides social work services in the home with children, youth and families. Facilitate Child and Family Team Meetings (CFT), create a safety plan with family, help family develop family vision and assist family with developing their Plan of Care to improve family stability. Integrate all involved vendors and service providers in the plan, including the family's formal and informal supports. Minimum Requirements: Bachelor's Degree in Social Work or related field with a minimum of one year direct service experience in the human services related field. Must possess valid driver's license, insurance, own vehicle, and have an acceptable driving record. Must have the ability to drive vehicle, travel to family's homes, move about in a family's home, climb stairs, work in an office environment, sit at a desk, use copier/scanner/fax, phone, possess computer skills, and operate technology (work issued cell phone, tablet/laptop and desktop). Preferred knowledge of trauma informed care and impacts of trauma on children and families. Clear all background checks and clearances. Must be able to work as a full-time employee. Ability to work effectively with clients, families, staff and community contacts from a variety of cultural and ethnic backgrounds.
