We are seeking a full-time and part-time person with experience & comfortable working with elders to help perform Transitional Coordination into independent living. Full-time will be based in Grand Junction, while the Part-Time person will work remotely from the Montrose/Delta area. Money Follow the Person Transition Coordinators (MFP-TC) shall have some knowledge in the areas of: Knowledge and experience of populations served by the MFP-TC and target population served by the service waivers Member interviewing and assessment skills Must have some local knowledge of available community resources and public assistance programs Transition plan development MFP-TCs Experience, Knowledge and Responsibilities Act as an advocate for the elders with disabilities Receive referrals from facility resident, family, friends, case managers, LTCD staff Obtain informed consent to participate in the MFP from the member or guardian Make a referral and/or find resources within supporting agencies Complete the transition Assessment/Plan in collaboration with the transition team If needed, assist the member with finding qualified housing prior to discharge (group home, assisted living apartment, subsidized housing or family home) Shop & purchase household goods and assistive devices with funds available through MFP Complete other activities necessary to establish safe community living Complete risk assessment tool to determine level of risk Complete all required documentation, reporting and monitoring responsibilities May assist our Community Choice Transition Coordinator with their member's needs Considerable knowledge of Medicaid, aging and disability related issues and services, and program planning Demonstrated ability to understand complex health care problems and organize strategies to address them Ability to review and evaluate the effectiveness of human service programs and to provide guidance to agencies and professional state and local management in preparing new and improved programs Working knowledge of person-centered planning ORGANIZATION CONFORMANCE STANDARDS FOR ALL POSITIONS Perform quality work within deadlines with or without direct supervision. Interact professionally and courteously with other employees, customers and partners. Work effectively as a team contributor on all assignments. Understand the necessity to efficiently and effectively interact, communicate and coordinate work efforts with other employees and organizations in an effective manner to accomplish common tasks. Function in highly stressful circumstances. Maintain a high level of professionalism and to conduct business in an ethical manner at all times. Maintain regular and punctual attendance. High school education or equivalent. Class C Driver's License or equivalent. Clean driving record. Ability to safely operate a wide variety of vehicles. Good communication skills (ASL and/or Spanish a plus) Pass a federal and Colorado background check with a fingerprint screening. Ability to pass pre-employment and random drug and alcohol screenings. Ability to navigate/willing to learn online billing/invoicing and reporting portal. The desire to learn new things and adapt to change.
