Direct Support Professionals (F/T & P/T & Per Diem) -- Our agency strongly believes that every individual deserves the opportunity to achieve their fullest potential. At the present time, we are looking for experienced persons to fill the role of Direct Support Professional. The right candidate will be responsible for providing compassionate care & assistance to individuals w/intellectual & developmental disabilities. DSPs should be caring, compassionate people w/dedication to create a better world for others, through both full-time, part-time & per diem positions available in our Residential programs. We are seeking Direct Support Professionals to work within the 5 boroughs of NYC. Positions are available in our Residential & Community programs. Responsibilities: - Assist with & work to strengthen the individuals' many daily living needs, including but not limited to: nutrition, hygiene, exercise, communication & socialization skills. - Develop a rapport w/residents & cultivate a safe & supportive relationship. - Encourage individual skill development & work towards community integration. - Develop meaningful relationships & connections w/individuals to make them feel cared for & included in the community. - Follow all health & safety protocol; including, planning & preparation of meals consistent w/ individual's diet & menu. - Keep a clean & tidy home for the individuals; including cleaning individual's room, kitchen, bathroom, closets, refrigerator, etc. - Update individual case notes on a daily basis. - Coordinate medical appointments & transport individual to doctor appointments, meetings, events & other activities as needed. - Use the person-centered philosophy throughout the day, which supports positive control & self-direction of individual's own lives according to the Dept. of Disabilities Services. Use each moment as a teachable moment. - Communicate important information to the supervisor & other members of the team. - Follow all agency procedures regarding incident reporting & reporting possible abuse. Cooperate during investigations & inspections. - Attend & participate in all scheduled training & meetings as requested by supervisor. - Responsible for complying w/rules & regulations governing access to Protected Health Information under HIPAA. - Perform other job-related duties as assigned by the supervisor. Qualifications and Requirements: - HS Diploma or GED required. - 1+ year professional direct care exp. or related exp. providing one-on-one care. - Can quickly manage stressful situations responsibly & effectively. - Flexible, w/keen ability to handle stress well. - Exp. tracking & recording information. - Ability to effectively communicate both verbally & in writing & work within a team setting. - Valid NYS Driver's License required Core Competencies: - Integrity & Responsibility - Confidentiality - Respect - Relationship management - Advocacy - Ability to promote physical & emotional well-being - Ability to promote the person-centered philosophy Work Environment & Physical Requirements: The physical demands & work environ. characteristics described here are representative of those an employee encounters while performing the essential functions of the job. - Regularly required to speak clearly & hear the spoken word well. - Regularly required to utilize near vision ability to read data and documents, including spreadsheets & reports, in printed form & on computer screens. - Regularly required to provide physical care for clients & individuals, which may include, but is not limited to bathing, feeding, changing clothes or diapers & pushing wheelchairs.
