Individuals would see same-day sick, follow up visits and share in panel management. Experience preferred. Established internal medical practices in Warwick and Smithfield, RI. Practices are actively engaged in the patient-centered medical home model and have high functioning patient care teams including nurse care managers, pharmacist, pharmacy techs, social work, and integrated behavioral health.
