Rate of pay commensurate with years of experience at GHV We offer Medical, Dental, Life Insurance, 403(b) Retirement, Holidays, and generous PTO POSITION SUMMARY This position is an essential member ofthe PROS (Personalized Recovery Oriented Services) program and agency management team, responsible for the operation and oversight of OMH licensed psychiatric rehabilitation program, in membership of internal and external committees and provision of consultation across departments. The PROS program provides individuals with severe and persistent mental health conditions a full range of comprehensive, recovery-oriented services, with the purpose of assisting them in overcoming barriers caused by their mental health condition, developmental disability or substance use disorder Seeking aLicensed Practitioner of the Healing Arts: Creative Arts Therapist (LCAT and CAT-LP) Credentialed Alcoholism and Substance Abuse Counselor (CASAC) Marriage and Family Therapist (LMFT or MFT-LP) Mental Health Counselor (MHC and MHC-LP) Nurse Practitioner, Physician Assistant Registered Nurse, Occupational Therapist Pastoral Counselor, Licensed Clinical Psychologist Licensed Clinical Social Worker Licensed Professional Clinical Counselor Licensed Marriage and Family Therapist Rehabilitation Counselor (CRC) Social Worker (LMSW and LCSW) Therapeutic Recreation Specialist a licensed-eligible practitioner registered with the Board of Psychology or Behavioral Science Board. ESSENTIAL RESPONSIBILITIES Program Operations Provide complete oversight of the PROS program including but not limited to, collaboration with central intake department, initial and ongoing assessments, and daily operations. Function as a leader and provide clinical expertise and innovation for areas of need for enhanced service provision in PROS program and throughout agency. Collect, interpret and utilize data driven outcomes to improve functioning and revenue of program and agency. Provide continued education on changes to managed care Provide an array of group psychosocial rehabilitation services designed to support individuals in restoring interpersonal, self-management and basic living skills; prevent relapse; manage symptoms related to behavioral health conditions; monitor/address physical health conditions; strengthen wellness and self-care practices and achieving person-centered goals. Coordinate/implement initial and ongoing psychiatric, psychosocial, health, substance abuse and psychiatric rehabilitation assessments. Conduct oversight and ensure compliance in accordance to OMH and Medicaid regulations and standards Conduct periodic reviews and updates of policies and procedures to comply with OMH and Medicaid regulations Plan, develop and implement evidence-based curriculum for groups Coordinate services among PROS components. Maintain on going communication with program psychiatrist and community providers regarding clinical treatment issues of individuals served in PROS and community-based clinics. Communicate with referral sources, individuals and families concerning referrals, assessments and coordination of treatment planning to ensure continuity of care. Oversee the discharge planning process. Identify the resources and supports needed for transition to another program and make necessary referrals, linkages for treatment, rehabilitation and support services. Participate in local advisory boards representing Gateway Community Industries, INC in addition to obtaining and sustaining relationships with community providers Staff Development Evaluate program outcomes and productivity of staff Hire, train, supervise and evaluate PROS Licensed staff Hire, train and provide clinical supervision to professional staff Plan, develop and implement trainings for staff development. Develop, facilitate and supervise the provision of intensive Rehabilitation services Oversee and mentor interns in PROS. Service Provision/Direct Care Collaborate with individual and collaterals in developing, reviewing and modifying an Individualized Recovery Plan that identifies the medically necessary services that will support the individual's goals and identified path to recovery. Maintain case records on each individual on case load in accordance with recognized and acceptable principles of recordkeeping. Ensure all documentation is completed in a timely manner and complies with OMH/Medicaid regulations and agency policies and procedures Develop and facilitate Intensive Rehabilitation groups. Provide goal-oriented verbal counseling or therapy, including individual, group and family counseling or therapy for the purpose of addressing the emotional, cognitive and behavioral symptoms of a mental health disorder or for engaging, motivating and stabilizing persons with co-occurring mental health and substance abuse disorders and the related effects on role functioning. Provide crisis intervention services de-escalation of situations of acute distress which require immediate attention and to address immediate risks such as relapse, hospitalization, loss of housing or involvement with the criminal justice system. Provide engagement services with the purpose of fostering an individual's commitment to enter into therapeutic relationships and participate in program services supportive of the individual's recovery. Provide family psychoeducation with the purpose of assisting and enhancing the capacity of a collateral to facilitate individual's recovery. Provide targeted, interventions necessary to address immediate risks such as relapse, hospitalization, loss of housing or involvement with the criminal justice system. Facilitate psych rehabilitation groups and services designed to develop or improve individual's personal coping skills, prevent relapse and promote recovery. Such services may include coping skills training, disability education, dual disorder education, medication education, problem solving skills training and relapse prevention planning. Monitor the ongoing effects of individual's course of care including the monitoring of individual's symptoms of mental illness, as identified in his/her Individualized Recovery Plan. Consul with identified collaterals and communicate with psychiatrist and/or nurse. Qualifications REQUIRED QUALIFICATIONS Education: Licensed Practitioner on the Healing Arts Experience: Three years' experience in direct service to individuals with severe and persistent mental health and/or substance use disorder. Two year's supervisory experience.
