Registered Nurse Job Category: Professional Requisition Number: REGIS005618 Full-Time Holyoke, MA 010403211, USA Job Details Description CHD Adult Mental Health Holyoke is looking for a Registered Nurse to work in an innovative intensive medical group home situated in Belchertown, MA. The RN will work closely with five persons, all of whom are referred by the Massachusetts Department of Mental Health, their supports (family members, friends, Peer Support Specialists), their primary care physicians, specialists and any other medical service providers to create and implement medical plans that address chronic medical conditions. Working with the Management Supervisor and Specialty Site Manager the RN will review referrals for appropriateness. The RN will work with the Persons referred to complete a nursing assessment and a medical plan to address chronic medical issues. The RN will also train, review, and/or coach persons served in the skills necessary to manage chronic medical issues e.g. measuring blood sugars or self-administering insulin. The RN, working with the Person served, any identified supports (Friends, Family, DMH Case Manager, Peer Specialist) and other staff will formalize the Person's community service plan goals, objectives and interventions. The RN will work with the Person served to achieve medical and medication related goals, objectives and interventions. Annual starting salary at $77,000, and range to commensurate with experience. The RN will also: Coordinate the exchange of medical and medication information among Persons served, staff, and extra-agency medical or nursing providers Consults and educates staff regarding any special care needs of the Persons served by the intensive medical group home Interact with Persons served in order to assess medical and other needs, provide information, discuss problems and encourage growth and development Provide informal counseling and education to Persons served in areas of health and psycho-physiological well-being Monitors and reports client progress in areas of health and psycho-physiological well-being Accompany Persons served to medical appointments and medication reviews as necessary Educates Persons served as to medication function and reports any effects or side effects to the prescriber Monitors and reports drug blood level to prescribers as requested Makes appropriate clinical interventions e.g. arranges appointments with physicians or prescribers Coordinates communication among health care providers serving each person served Qualifications Behaviors Preferred Enthusiastic: Shows intense and eager enjoyment and interest Innovative: Consistently introduces new ideas and demonstrates original thinking Motivations Preferred Flexibility: Inspired to perform well when granted the ability to set your own schedule and goals Education Preferred Bachelors or better.
