About the Role: The Transgender and Non-Binary (TGNB) Integrated Health Navigator will provide assistance to transgender and gender non-binary (TGNB) patients in navigating medical and supportive service systems related to gender affirming surgery. General responsibilities include: Perform assessments and develop a personalized care plan. Develop relationships with community organizations supporting TGNB communities. Conduct outreach activities to include community visits and surgery consults. Follow up appointments and at home visits. Assist in the development of health education materials focused on the health needs and concerns of TGNB communities. Work Schedule/Benefits: Callen-Lorde is open Monday - Saturday. This is a Monday-Friday work schedule, and some evening hours will be required. Qualifications Direct TGNB service experience or relevant TGNB life experience. Experience in social services, case management and/or entitlements work preferred; specifically with TGNB, LGB, and HIV-affected communities. In-depth knowledge of health and social issues facing the TGNB community and familiarity with service and resources available. Knowledge of transgender surgical procedures, WPATH standards of care and NY state TGNB health insurance requirements preferred. Bilingual (English/Spanish) preferred. Personal Characteristics, Skills, and Abilities: Ability to work with diverse populations, including: communities of color, immigrant communities, TGNB and LGB communities, people living with severe and persistent mental health diagnoses, PLWHA and people with addiction histories. Ability to work independently while being team-focused. Strong communication and interpersonal skills. Ability to remain calm, focused, and helpful under stress; ability to multitask.
