We are currently seeking a full-time Assistant Director of Social Work, who will embody the passion and commitment required to deliver the ultimate care for our residents. Duties will include the provision of direct resident care and the supervision of a fully staffed dynamic Social Work Department. This position is scheduled Monday-Friday from 8:30 am - 5:00 pm. REQUIREMENTS INCLUDE: MSW from an accredited School of Social Work. Licensed Masters Social Worker (LMSW) or a Licensed Certified Social Worker (LCSW) with the New York State Education Department. Minimum 5 years post graduate experience within a hospital or long-term care setting preferred. Experience in casework, group work and family therapy preferred. Minimum of 4 years supervisory experience preferred. Strong knowledge base of CMS/DOH Rules and Regulations. Strong knowledge base of Medicare/Managed Care/Medicaid/Finances. RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO: The provision of direct resident care and the supervision of a fully staffed dynamic Social Work Department. Meeting new admissions and their families upon admission. Completion of psychosocial assessments, PHQ2-9 interviews, and creation of comprehensive care plans. Assisting residents and their families with the adjustment to placement, health needs/concerns, bereavement support, and concrete services. Attending weekly Interdisciplinary Comprehensive Care Plan Meetings, Rehab Review Meetings and discharge planning meetings. Working with residents with dementia, mental health/behavioral needs and a variety of health care needs (including vent/trach). Acting as a liaison with community entities and the Gurwin Center: hospital systems, homecare agencies, assisted living facilities and hospice organizations. Completion of MDS+, PDQ2-9 interviews, PASARR SCREEN's, and other evaluations as required by regulation. Assisting families/representatives with the completion of Health Care Proxies, MOLST (Advanced Directives), work collaboratively with Hospice providers, and provide end-of life care to residents and their loved ones. Discharge Planning: assess the resident's medical/psychosocial needs to facilitate a safe and appropriate discharge. Provide residents families with concrete services and referrals. Managing social work department in collaboration with and during the absence of the Chief Guest Relations Office/Chief Social Work Officer. Orientation and training new employees. Creating personal improvement plans (PIP) when needed and completion of employee evaluations. Assist and guide staff with daily challenges: cases, understanding financial impact/ramifications, referral sources, and concrete services. Important skill sets: crisis intervention, mediation, an understanding of systems dynamics within a healthcare setting, and a lot of common sense.
