Vocational Case Manager New York, NY ( Workforce Job Type Full-time Description The primary function of theBi-Lingual Vocational Case Manageris to work with clients to ensure their smooth transition into the workplace.The vocational case manager will facilitate a portion of the job readiness training classes. The vocational case manager will be responsible for overseeing the data entry in GMHC's data base as well as the data entry required sources. The vocational case manager will be responsible for meeting with the job developer, job readiness training instructor and other job readiness staff to ensure client's success in securing and retaining employment. Essential Job Functions The following duties are mandatory requirements of the job: Meet with clients on an ongoing basis to discuss and support client in employment related issues. Complete assessments, reassessments, transitional benefit plans and care plans for active clients Provide active referrals to internal and external providers to assist with job placement. Maintain and input data entry required by GMHC, and other contracts, as required. Complete employer site visits to assist in the support of clients and continuation of care and retention. Assist with special projects and perform other related duties as assigned by supervisor Requirements Education and Certification Bachelor's degree required in Social Work, Human Services, Social Science, or Communications Minimum of 3 years related experience Special Skills and Knowledge In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all the following: Bilingual in English/Spanish preferred Excellent written, verbal, and interpersonal communication skills Strong presentation skills with the ability to speak to large groups Experience working with clients living with HIV/AIDS preferred Intermediate knowledge of Microsoft Office Suite; Excel, Outlook, etc. Demonstrated experience in case management with diverse clients, with barriers to employment in jobs. Experience in working with current/ former substance users in a social service organization performing case management services. Salary Description $45,000 - $48,000 per year
