Per Diem Nurse - LPN Job Details Level Experienced Job Location Good Shepherd Community Care Hospice Residence - Wayland, MA Per Diem Nurse - LPN Recently named by The Boston Globe as one of The Top Places to Work in Massachusetts, our community based, mission-driven hospice and palliative care organization seeks a Per Diem Nurse or LPN to join its expanding hospice team. Our ideal candidate is able to fill in shifts at the Residence and/or do continuous Care shifts. Our Hospice Residence is located in Wayland and was reopened in Fall 2022. This is an exciting opportunity for LPNs interested in providing end-of-life care at a new 10-bed hospice facility. Care is also provided in patient's private homes and residential facilities throughout Greater Boston and parts of Metrowest. Travel is required for this position. We will help make the commute easier for you. For both continuous Care and the Residence, the shifts are 12 hour shift. Job Responsibilities: This position provides direct patient care including on-going assessment, pain management. and symptom control in line with the plan of care established for the patient. The Shift Nurse notifies the Nursing Care Coordinator of any significant change in patient's condition or any changes necessitating physician notification and documents all care provided including clinical observations and interventions. Education, Skills and Experience: Current Massachusetts LPN License in good standing. Minimum of 2 years experience in community health, oncology nursing, or a related field preferred. Comfort working independently in a one on one setting with patients.
