Job Description Position Title: Licensed Practical Nurse (LPN) FLSA STATUS: Non-Exempt REPORTS TO: Nurse Manager SUPERVISES: N/A DEPARTMENT: Home Health COMPANY: Overlook Home Health Services POSITION SUMMARY : The LPN provides individualized nursing care and comprehensive services to patients and families in their own homes under the supervision of a Registered Nurse. PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS : The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned. Provides basic patient care under the supervision of a Registered Nurse in accordance with physician's orders Observes patients, charting and reporting changes in patient's conditions, such as adverse reactions to medications or treatment and taking any necessary action Educates patient and families in self-care and health management Attends case conference as requested Adheres to policies and procedures Participates in agency quality management and safety activities QUALIFICATIONS : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Educational Requirements and Experience: Graduate of an accredited school of nursing Current license to practice as a practical nurse in the state of Massachusetts Current CPR required Two years nursing experience Current MA driver's license and reliable transportation PHYSICAL REQUIREMENTS/WORKING CONDITIONS : Works primarily indoors in varying climate controlled settings Possible exposure to unpleasant odors Possible exposure to chemicals as identified in the MSDS Manual Subject to hostile and emotionally upset family members, personnel and visitors May work beyond normal working hours and on weekends and holidays when necessary Required to travel to multiple work sites and up to 200 miles per day COMMUNITY OVERVIEW: The Overlook is a non-profit organization operating since 1911 and offering a full continuum of care and services for older adults in MA.
