At Ability Beyond, you can be accepted, celebrated, & impactful We are seeking a Licensed Practical Nurse (LPN) to join our team Working closely with the Nursing staff, the Professional Supports Licensed Practical Nurse (LPN) is responsible to provide administrative and clinical supports to and for the individuals within group home settings. Location: Beacon, NY Pay Rate: $33.50 / hour Responsibilities: Hires, supervises and oversees development of Medical Support Assistants (MSAs). Facilitates collaboration/communication among direct reports, supervisor and support personnel to ensure that the health care needs of persons served are met. Tracks and ensures that all necessary clinical appointments, including those for preventative care and diagnostics, are completed, collaborating with residential supervisors, and Team Leader - Nursing as appropriate, when scheduling issues arise. Ensures Ready to Go Books are maintained; necessary clinical information is available and communicated to health care providers and relays outcomes of appointments to the Interdisciplinary Team. May attend medical appointments. Ensures receipt of clinical documentation, e.g., laboratory and other diagnostic test results, hospital and consultation paperwork. Completes clinical documentation, including but not limited to Health Care Reviews, Nursing Daily Documentation Forms and medication/treatment transcription. Ensures Medical Book management. Enters clinical data, including but not limited to allergies, diagnosis, laboratory and diagnostic test results into Evolv. Performs clinical audit activities sharing outcomes and collaborating with Primary Care Nurse and/or the Team Leader - Nursing and the residential team to address areas of concern, including problematic clinical findings and lapses in documentation. Performs direct care, including but not limited to injections, dressing changes, tuberculin testing, as assigned within LPN scope of practice. Assists with ordering/tracking medications, enteral feedings and other clinical supplies and equipment, and facilitates maintenance of the latter as assigned. Travels to other NY group homes, no less than one time per week as required. Qualifications: Graduate from an accredited nursing program. Must have a current LPN license to practice Nursing in the state of NY. Must be proficient in the delegated nursing skills administered by the assigned Registered Nurse. Travel required to homes or appointments Required to become CPR certified within three (3) months of date of hire.
