JOB SUMMARY: The Limited English Proficiency (LEP) Program Coordinator will assist with the coordination and provision of case management services to eligible clients who indicate during the intake process that they speak English less than 'well'. The LEP Program Coordinator will provide targeted Outreach to Haitian Creole, Chinese, and Spanish speaking communities (the top languages identified in New London County), with the goal of increasing awareness of the Agency and accessibility of resources to those populations. This position will also provide universal intakes, assessments of strengths and needs, connections to TVCCA programs, application assistance for DSS programs, and information/ referral services to LEP eligible clients in need of general case management services. The LEP Coordinator will be responsible for following up on all referrals as well as providing ongoing, outcome-oriented case management. This position must utilize TVCCA's TribeWare case management software and become knowledgeable of the Results Oriented Management Accountability (ROMA) cycle as well as the Human Services Infrastructure service model. TVCCA operates under these two management principles and all employees must understand and integrate them. Experience providing strength-base case management services to singles and families. Demonstrate a history of working with minimal supervision in a mature manner. Excellent computer skills (Microsoft Office Suite: Excel, Word, Outlook). Strong communication skills. Must be bi-lingual. Possess a valid driver's license and daily access to a reliable and insured motor vehicle Required Experience
