Qualified Intellectual Disabilities Professional (QIDP) Department Residential - Hudson Valley Type of Position Full time Search Location(s) Carmel, NY, Mahopac, NY, Patterson, NY, Pawling, NY, Wappinger Falls, NY Apply Now ( Job Brief The QIDP coordinates all aspects of care for residents in order to ensure active treatment is provided on an on going basis in the least restrictive environment, in accordance with the OPWDD. Shift Worked: Sun-Th or Tu-Sat, 11a-7p with flexibility Weekly Hours: 37.5 FLSA Status: Non Exempt Pay range: 50,000-55,000 The QIDP coordinates all aspects of care for people we serve to ensure active treatment is provided on an ongoing basis in the least restrictive environment, as per the OPWDD. The QIDP provides oversight, coordination, supervision, and evaluation of operations of assigned programs/sites, including site supervision for therapeutic or counseling professionals that provide services. Physical Expectations This position requires that employees are able to stand and or sit for prolonged periods of time. The employees must also be able to lift and transport consumers and perform all physical aspects of the job. Essential Duties and Responsibilities include but are not limited to the following Coordinate and conduct Annual and Semi-Annual reviews of consumer status/progress including an assessment of risks to consumer's rights and protection. Ensure completion and receipt of individual clinical team members' reports/meetings evaluations two weeks prior to schedule ITT meetings. Ensure the follow-up of recommendations stemming from reviews/special meetings. Prepare annual CFA's and summaries which include a capabilities needs assessment, priority of needs and a summary of clinical reports, activities schedules and consumer profiles. Ensure at least restrictive interventions or practices are utilized. Ensure individuals identified as having practices, interventions and or adaptive equipment, which may pose a risk to consumers' rights and or protection, are presented to the Human Rights Committee for their review and or approval. Prepare semi-annual and annual ITT reports consisting of programming. Develop and implement goal programming and activity schedules. Monitor data collection on an ongoing basis. Provide annual and semi-annual goal progress reports. Observe Program Specialist conduct goals active treatment with and provide hands-on-training as needed or prescribed consumers by facilities POCA. Provide staff with in-serve training ongoing. Ensure integration of services between Day Program and the residence and, integration of all services into routine daily activities. Complete CSET schedule annually and update as needed. Also ensure, CSET training is provided and reported on by clinicians as needed. Complete Quality Improvement reports as needed and ensure resolution to any outstanding issues. Ensure all interactions support the core values and mission of the Agency. Maintain communication with family/correspondent in regards to residential programming in conjunction with Social Worker. Participate in ICF - IPR and IUR audits. Additional responsibilities as assigned as needed by Deputy Director and/or Director, e.g. committee participation, in-service training for the agency, attendance at special meetings, etc. JOB QUALIFICATIONS Education: Bachelors Degree in related field of study required. Required Experience: At least one year's experience in the field. Well-organized, experience writing formal goals.License: Valid Drivers License and Agency Insurability.
