Overview: We are seeking a dedicated and compassionate Care Manager to join our team under the general supervision of the Director of Senior Services. In this role, you will provide essential care management services to individuals participating in our program. For the ideal candidate, we offer flexibility on whether this will be a full-time or part-time position, depending on mutual agreement. The compensation range is $30 - $35 per hour. Responsibilities: As a valued member of our team, your responsibilities will include (but are not limited to): Assisting clients both on-site and in their homes to access support services, including entitlements, medical care, legal services, senior housing, educational and social programs in the community Preparing and maintaining accurate records in accordance with agency policies and procedures. Participating in agency committees and special projects, as needed. Engaging with the community by attending meetings with other agencies serving the elderly, as appropriate. Performing other duties as assigned. Requirements: We are seeking candidates who possess the following qualifications: Masters Degree in Social Work, Mental Health Counseling, or related field from an accredited school. At least two years of experience in a Care Manager role, or similar position. A valid driver's license and access to a reliable vehicle.
