Community Health Worker Primary Care- Third Street, 8 East 3rd St, New York, New York, United States of America Req #1200 Monday, September 9, 2024 Title: Community Health Worker Salary: $50,000 Position Overview: The Community Health Care Worker enhances traditional care teams and builds individual and community capacity by increasing health knowledge and self-sufficiency through range of activities such as outreach, community education, informal counseling, social support and advocacy. The Community Health Worker will be responsible for helping individuals navigate to access community services and other resources. This position includes no managerial or supervisory responsibilities. Essential Duties & Responsibilities: The essential duties of thePositioninclude but are not limited to the following activities: Support linkage to care and treatment adherence by educating clients about the appropriate use of services and current medication options for treatment. Support linkage to and retention in care by re-engaging clients lost to follow-up. Help individuals, families, groups and communities develop their capacity and access to resources, including health insurance, food, housing, quality care and health information. Facilitate the development of communication and client empowerment to interact with healthcare/social service systems. Serve as a liaison between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Support adherence to treatment by providing structured educational sessions on specific topics such as chronic diseases, HIV, viral life cycle, treatment, and side effects providing an understand health condition(s) and develop strategies to improve health and well-being. Deliver health information using culturally appropriate terms and concepts. Link individuals to health care/social service resources. Provide informal counseling support and follow-up. Support retention in care and reduce barriers by collaborating with medical, behavioral health, and social services providers. Document activities in electronic health records. Qualifications: High School Diploma or GED required. Associates Degree in Social Services, Human Services, or Public Health preferred. 2-3 years of related experience working in a community -based setting. HIV/AIDS/STI knowledge including prevention, infection, and treatment. Knowledge of healthcare terminology. Basic computer skills for word processing and spreadsheet use. Good communication skills. Ability to work in a culturally diverse environment. Lived experience preferred. Bilingual - Spanish a plus. Physical Demands & Working Environment: Most work is completed in the community, with occasional work completed in office. Not substantially exposed to adverse environmental conditions (Typical of most clerical and administrative positions). Sedentary work equivalent to lifting approximately 10 pounds with occasional lifting and/or carrying of small objects Frequent and extended walking and standing. Seldom bending, climbing, reaching and lifting. Follows clearly defined policies and procedures. Makes decisions within well-defined parameters for the completion of own tasks only. Supervision: Routine supervision: Provided with instructions on job requirements, methods, priorities and deadlines with particular attention to unusual or difficult aspects or tasks. There are regular or systematic reviews of progress, quality and output, usually at discrete times, but occasionally only upon completion. All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
