CASE MANAGER SS Job Details Level Entry Job Location New Paltz Family Health Center - New Paltz, NY Position Type Full Time Education Level Not Specified Salary Range $21.77 - $21.77 Hourly Job Shift Day Job Category Health Care Description Summary: Case managers screen adults, children and families for insurance and other benefit program eligibility. Case managers complete applications for eligible families and provide intensive tracking and follow-up with applicants to ensure that benefits are successfully accessed and all required supporting documentation is obtained. Qualifications Qualifications: Works collaboratively with physicians and other health care personnel in patient evaluations and treatment to further their understanding of significant social and environmental factors underlying patients health problems. Provides direct case management services to assist patients in receiving necessary resources and services such as: Medicaid, Child Health Plus, Welfare, DAS, Social Security, Child Care, Home Care, Housing/Shelter/Safe Homes, Substance Abuse Treatment, Mental Health Treatment, HIV Services, Legal, other Health Insurance etc. Completes sliding scale, 340 B pharmacy benefits, financial waiver, and cancer service applications as needed. Helps assist patients with insurance options including pooled trusts. Documents all patient services in patients electronic medical record, and closes encounters within 24 hours. Prepares case management assessments, patient histories, service plan and reports. Advocates when necessary for patients with outside systems.
