EAC Network, a leading not-for-profit social service agency serving over 62,888 people through 100 programs across Long Island and NYC, is looking for a dedicated Health Home Plus Care Coordinator to join our Nassau County Long Island Health Home Plus Program. Competitive Compensation:$28 per hour with a hybrid work schedule. Your Impactful Role: Lead the Way:Coordinate care for clients with severe mental illness, substance use disorders, and chronic medical conditions, ensuring they receive comprehensive support. Make a Difference:Conduct outreach, assessments, and service planning to help clients achieve their health goals and maintain contact with their support networks. Fieldwork & Flexibility:Engage in home visits, community work, and hospital visits to ensure clients are connected with the services they need. Collaborate for Success:Work with a multidisciplinary team, including case managers and treatment providers, to create and implement effective care plans. Monitor & Support:Track client progress, assist with crisis interventions, and ensure follow-up on medical directions and aftercare discharge. What We're Looking For: Qualifications:Master's degree in a human services field with one year of experience, or a bachelor's degree with two years of experience, or a CASAC with two years of experience. Alternative qualifications include three years of experience in a relevant field or two years as a Health Home care manager. Experience:Direct service experience with serious mental illness, developmental disabilities, substance abuse, or children with serious emotional disturbance is required. Skills:Excellent organizational, time management, and communication skills. Proficiency in Microsoft Office and other relevant software is necessary. Other Requirements:Valid NYS driver's license and reliable transportation, with proof of insurance and a Defensive Driving Certificate within 30 days of hire. Why Join Us?
