FamilyAid Boston, the city's largest human service agency solely focused on ending childhood and family homelessness is seeking a Crisis Shelter Case Manager Housing Support to provide housing search, placement and stabilization case management incorporating two-generation services to families residing in FamilyAid's short term crisis shelter program. To support these strategies, the agency seeks a dynamic, driven Bilingual Case Manager, Community Based Shelter Program, to support the agency?s deepening 2Gen approach to serve to more than 5,000 children and parents. The Case Manager, Community Based Shelter Program will provide supportive services to a caseload of families living in a shelter setting, as well as to families who have recently moved from shelter into permanent housing. Will develop and implement plans that assist families transitioning from shelter to permanent housing within guidelines established by the program and the contract holder (The Department of Housing and Community Development). The Case Manager will assess families for strengths and needs, will maintain a caseload of families participating in the shelter and stabilization programs and will provide ongoing case management to promote eventual stability in permanent housing. Will conduct regular home visits, as required by client needs, program requirements, and/or contract guidelines to support families. Will develop plans for rehousing and program participation according to contract expectations. The successful candidate will have a bachelor?s degree. Master?s degree in Social Work is strongly preferred with experience in social services. Ability to work in office and field-based settings with a diverse, homeless population. Work Schedule and Conditions: Full-time, hybrid work schedule with flexibility to best meet client and program needs. Some evening availability required.
