High School Diploma and 1 year of experience working in a mental health setting OR a Bachelors Degree in Human Services. Clean valid NY State Drivers License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health. JOB FUNCTION: Provide supportive counseling, restorative services, and crisis intervention training in a licensed residential program providing transitional housing to people with psychiatric disabilities. ESSENTIAL FUNCTIONS: Provides supportive counseling, restorative services and appropriate behavioral intervention to residents on a daily basis and completion of such documentation as per agency standards on a daily basis. Meets weekly with assigned primary residents to establish, work on and review short and long term service plan goals as required. Maintains collateral contacts with outside service providers at least monthly. Assist residents in procuring financial entitlements as often as necessary each month. Monitor medication for residents as necessary on a daily basis and adhere to the programs policies and procedures regarding medication management. Supervise and assist residents in completing all household chores and ensures that all living spaces are kept clean and orderly daily. Management of crisis situations under supervision of Program Supervisor. Alerts Senior Counselor and/or Program Supervisor to maintenance/safety issues as they arise daily. Conduct fire drills at the residence and document such drills on a monthly basis. Report all incidents to Program Supervisor and documents incidents as per agency policies/procedures. Actively participate in weekly staff meetings and monthly High Risk Meetings. Provides transportation to residents as often as needed each week. Comply with agency Compliance Plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description. Utilize all agency-based IT applications in a proficient manner including collaborative documentation, review of emails at least twice per shift (at start and prior to close of the shift), as well as utilizing Outlook Calendar on a daily basis. Remain current on changing developments in individual field of expertise and licensing credentials. Perform all other job duties as assigned. COMPETENCIES: Ability to relate to residents in a positive and supportive manner. Understanding of restorative services and ability to provide such services in a clear manner. Understanding of psychiatric disabilities and dial-diagnosis. Ability to utilize a team approach to problem solving. Good written and verbal communication skills. Good computer literacy skills. Ability to multi-task. Good interpersonal skills. Good organizational skills. Willingness to accept supervision and utilize learning opportunities.
