Job Summary: Work on site in hospital serving as primary liaison of the delivery @ discharge program. Assist pharmacy staff in the triaging discharge prescriptions, the delivery of filled prescriptions to the patients and collecting payment at the time of delivery.Job Duties: Identify and enroll patients to the discharge and delivery programsServe as on-site liaison to hospital doctors, nurses and discharge coordinators to drive utilization of the delivery @ discharge program.Proactively identify opportunities for increased efficiencies as well as potential barriers to delivery @ discharge programWork closely with home pharmacy to ensure timely delivery of discharge prescriptionsHighlight Kinney Drugs Rx services to discharge patient to aid in new customer retention Executes the plan - Able to learn and understand specific aspects of the pharmacy management system (i.e., adjudication queue) as well as our POS system; follows instructions and completes tasks with a sense of urgencyQuality focused - understands and embraces the importance of our Quality Assurance Policy & Procedures; understands and embraces all patient care programs and initiatives; understands and embraces all policies and procedures; accuracy with cash and related cash controlsAssist in the Triaging of discharge prescriptionsVerify and gather necessary patient information including name, address, phone number, date of birth, allergies, chronic conditions, request for non-safety caps and insurance informationAssess the workload, establish the prescription pick up time and queue the prescription for processingAssist in the delivery of prescriptions to the patient, including obtaining patient signaturesProvide quality customer service to all customers Maintain confidentiality regarding all patient information Must keep current with all State and Federal requirements as related to conditions of employment Responsible for completing all mandatory and regulatory training programsPerform other duties as assignedEducational Requirements:Minimum: High School Diploma (or currently enrolled) or GEDPreferred: AS Degree or Higher Required (Vermont Employees):Registeredwith the State of Vermont as a Pharmacy Technician Experience:Preferred: 3 years Hospital or retail pharmacy experienceSpecial Conditions of EmploymentDrug testInitial and continuous exclusion and sanction/disciplinary monitoringAny and all additional eligibility requirements based on the specific positionCompensation: $15.00-17.00 per hourThe final offer will be determined after careful consideration of multiple factors such as relevant skills, years of experience and education.KPH Healthcare Services, Inc. is a multistate organization and abides by all local, state and federal regulations as it pertains to minimum wage requirements.
