They gather information on medical, psychological, and social factors affecting the client's situation and evaluate their capabilities. The Case Worker provides counseling and support to help clients develop plans to meet their needs, mobilize internal and external resources, and improve their social functioning. They monitor client progress, refer them to community resources, maintain accurate documentation, and ensure effective communication with supervisors and funding sources. Additionally, the Case Worker may assist with housing individuals and providing necessary supplies and support to help them maintain their housing stability.This Position Pays $21.00 Per Hour and is Full Time at 35 Hours a Week.Education and/or Experience Required at Entry:Associate Degree in Human Services or a related fieldThree years of experience working with vulnerable individualsEquivalent combination of education and experience will be consideredCandidates with lived experience are preferred and encouraged to applyLicenses/Certificate/Clearances:Valid N.Y.S. Drivers LicenseRegistered and insured vehicleN.Y.S.
