The Clinical Coordinator develops and maintains clinical pharmacy services to ensure the highest level of patient care and safety. Responsible for development and implementation of clinical programs, policies, and procedures. The Coordinator provides oversight of pharmacy practice programs, medication use systems, and quality initiatives to detect, mitigate, and prevent medication adverse events and works to ensure medication use is aligned with evidence-based practice and regulatory standards.TeamworkInvolves other departments in project planning and implementationSupports others in the Pharmacy department and provides help and assistance when possibleRegularly attends and participates in team/or departmental meetings and activitiesKeeps others in the department informed of relevant informationRegularly offers and requests constructive feedback and coachingEstablishes and maintains effective working relations KEY RESPONSIBILITIES:Develop, promote, maintain, and evaluate patient care services to achieve appropriate care standards, quality improvement and cost savings.Review all non-formulary requests and provide overall formulary management. Develop and maintain and ongoing, systematic drug utilization evaluation (DUE) program.Work with interdisciplinary programs to deliver medication therapy management within patient care services and to ensure pharmaceutical care programs are appropriately integrated throughout the hospital.Develop and implement clinical programs, policies, and procedures.Collaborates with pharmacists, physicians, and other healthcare professionals to optimize patient outcomes.Participates in performance improvement activities using evidence-based practice.Summarize and assist in the documentation of reported adverse drug reactions; develop and maintain a surveillance program for adverse drug reactions.Attends daily rounds in ICU and actively participates in Antimicrobial StewardshipActive member of the Pharmacy & Therapeutics Committee.Disseminate information through intermittent distribution of department, committee or hospital communications/newsletters.Collaborate with the Pharmacy Enterprise Group to establish system-wide clinical initiatives.Establish and maintain a staff development program.Coordinate in-service education, drug information, competency validation, and other educational services for medical and nursing staff.Enhances professional growth and development through participation in educational programs, in-service meetings, workshops and professional affiliations.Serves as a pharmacotherapy expert and resource.Work closely with Nursing Education to review medication variances and falls.Review and update order setsAssist with Drug Library on Smart PumpWork closely with the Director to be a cohesive Management Team.Perform other related duties as assigned or requested by the Director of Pharmacy.
