Develops interventions with focus on crisis de-escalation, improving access to services, and linkage to treatment, if necessary. When needed, conducts mental health screening assessments and makes appropriate interventions and referrals. When needed, provides a full range of mental health treatment services, including: Mobile Outreach/Screening, responding to hotline calls, assessments, crisis counseling, case management and participates in interdisciplinary treatment planning, record keeping, consultation and education. Provides psychoeducation to individuals in crisis and advocates for their overall wellbeing.QualificationsWork requires the knowledge of theories, principles and concepts normally acquired through completion of a Master's degree in social work, psychology or closely related field; one year of experience in a psychiatric setting plus three to six months of on-the-job training and orientation or Bachelors degree and two to three years of previous experience plus three to six months of on the job training. Valid driver's license with no pending violations and an acceptable driving record as stipulated by SJH policy and, if applicable, the SJH insurance carrier, for those required to drive a hospital vehicle. Work requires the analytical ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems.Work requires the ability to prepare detailed assessments of patient referrals, counseling individual, group or family members, or perform other duties requiring a comparable level of communication skill.Work requires providing direct, age specific patient care as appropriate to treatment setting.
