Entity: Penn Primary Care and Specialty Practices (PPC/PSP) Department: OB/Perinatal Location: Exton, PA Hours: Full-Time, Per Departmental Needs Summary: The Nurse Navigator provides clinical navigation support for identified patients within the Penn Medicine Health System. Performs clinical and administrative functions to support the coordination of outpatient care needs. Patient care includes care to OB patients in the following age groups: adolescent/adult (Menarche through Menopause). The Nurse Navigator will collaborate with Nursing staff, Physicians, and Practice Leadership to report care gaps and facilitate in minimizing those gaps. Credentials: Registered Nurse - PA (Required) Certification in related specialty (location specific) required within 3 years of hire; ACC RNC-OB or ANCC AMB-BC (Required) Education or Equivalent Experience: Bachelor's in Nursing, Healthcare Management, or related field (Required) 3+ years' experience as a clinical nurse (Required) MSN from an accredited school of nursing and 5+ years' experience as a clinical nurse (Preferred) We believe that the best care for our patients starts with the best care for our employees.
