In this role, the successful candidate We have an exciting opportunity to join our team as a Peer Counselor. The Peer Counselor will play a critical role in our Enhanced Treatment and Recovery (EnTRy) program. The EnTRy program is a new and innovative program designed to improve early detection and treatment of mental illness, particularly psychotic-spectrum disorders, across the lifespan in Sunset Park, Brooklyn as well as neighboring areas. The Peer Counselor works with patients in the EnTRy program, engaging patients in wellness activities, and helping patients increase their ability to self- manage their own health problems. As a central part of the Program, the Peer Counselor will lead or co-lead several WRAP or WSM groups; promote the EnTRY program, speak to and enroll patients into the EnTRy program; help patients connect to Case Management, Psychiatry, or Counseling sessions; perform telephone outreach or reminder phone-calls; and participate in team meetings. Duties will be performed both onsite as well as remotely from home or a designated locationJob Responsibilities:Educate patients about the importance of gettingboth medical care and mental health care, and attending all Programappointments.Educate patients about the EnTRy program,services offered, and answer questions about the program.Outreach patients by telephone on a regularbasis to check how participants are doing, and to ensure that they engaged andmotivated to attend the programReceive a list of patients who are not attendingappointments, or have upcoming appointments, and call or send letters to help re-engagethose patients.Lead or Co-Lead Wellness Recovery and ActionPlan (WRAP) groups and/or Wellness Self Management groups (WSM). When one cycleof WRAP or WSM is complete, the Peer Navigator will repeat the group with newpatients.Assist patients with information about communityresources, such as basic needs, vocational, educational, legal, and housingservices, and connect the patient to the Case ManagerProvide support to the Community Based MobileTelehealth TeamParticipate in regular team meetingsMinimum Qualifications:To qualify you must have a To qualify you must have a NYS certification as a Peer Specialist or an ability to obtain certification within one year of hire. Demonstrated interpersonal communications skills and the ability to empathize with, relate to, and effectively work with participants of mental health services. Active participation in mental health self-help activities, peer support, peer advocacy programs, participant-run organizations, or similar experiences. Willingness to work outside of office setting, in the community and homes of participants. The salary range for the role is $41,137.15 - $44,520.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
