Provides medically prescribed services to patients who have speech and language problems. Advises and consults with the physician, patient, family/significant others, and Agency personnel who share responsibility in the patient's care. Participates in the development, implementation, and evaluation of the speech therapy treatment plan.Administers diagnostic speech and language evaluations and completes basic informal audiological assessments. Prepares a report of each patient's diagnostic evaluation.Develops and implements the patient's plan of treatment in consultation with the attending physician, and when indicated, appropriate Agency staff.Develops a patient plan of care for speech therapy services.Plans with the patient, significant other(s) and physician for care which is feasible within available physical, financial and support resources. Counsels the patient and significant other regarding the patient's speech and language disturbances.Assists the patient/significant other(s) to accept responsibility for participating in the patient's care.Observes and reports to the attending physician and other Agency staff, at least every 62 days, the patients’ reactions to treatments and changes in the patient's condition or plan of care. Promptly notifies the physician of any changes in the patient's condition which suggest the need to alter the plan of treatment. Informs the Supervisor of Clinical of all such communication with the physician.Communicates with other disciplines involved in the patient's care and documents communication in the patient's record. Participates in multidisciplinary conferences.Identifies the need for service of the other therapeutic disciplines and initiates the appropriate referral after consultation with the Supervisor of Clinical Services and with the approval of the physician.Evaluates performance of those for whom he/she has supervisory responsibility through conferences, record reviews, joint home visits and written evaluations.Ensures timely completion of supervisory visits.Reviews educational materials and selects appropriate materials for Agency resource file/.Shares patient insurance information with the Director of Finance to ensure that care is provided within the financial parameters established for each patient. At the request of the Director of Clinical Services make patient visits to provide skilled nursing care.Ensures completion of daily/weekly reports for collection of statistical data.Participates in on-call rotation.Participates in activities relative to improved organizational.Assumes responsibility for continued professional growth.Foster positive image of Agency in the community-at-large.Maintains current CPR.Graduate of a N.L.N. approved school of nursing. A baccalaureate degree in nursing is preferred.One-year supervisory experience, preferably in a home health or public health setting is preferred.Three years recent nursing experience.Current license to practice as a registered nurse in Pennsylvania.Knowledge of third-party payor reimbursement guidelines.Ability to establish and maintain positive working relationships.Ability to exercise independent judgement.
