We invite you to join our compassionate and dedicated team making a difference in the lives of our members at the RiverSpring Certified Home Health Agency.RiverSpring Certified Home Health Agency provides the acutely ill patient with short term skilled home visits: nursing, physical therapy, occupational therapy, speech therapy, and social work.Under the direction of the Director of Nursing, the AL - Nurse Manager will be responsible for the coordination of the healthcare and wellness program as well as the personal care services to residents of the RiverSpring Assisted living or Adult Care facilities. This position is responsible for clinical assessments, oversight, and implementation of the approved individualized service plans as developed by the resident/family and physician to maintain the resident safely within the facility. This position will be Sunday through Thursday or Tuesday through Saturday 9:00am-5:00pm Must be Flexible. Ideal candidate will be eligible for a $5,000 (less taxes) sign on bonus.The AL-Nurse Manager will ensure that the plan is appropriate and responsive to the resident’s health care needs; as well as reevaluate clinical assessments and make any necessary changes to the plan. Provides oversight and supervision to the program nurses and home health aides provided by contracted agencies.Duties & Responsibilities:Responsible for the healthcare implementation of health-related services of the ALR and ALP.Participates in assessment and screening of applicants for appropriateness in accordance with policy, standards, rules and regulations.Participates in policy and procedure development, making recommendations to the Assistant Director of Nursing and Director of Nursing as necessary.Provides assessments and reassessments for residents, assesses health needs and identifies the appropriate level of care required by the individual. Documents as needed on resident care or changes in status.Assist in the development and implementation of the individualized service plan (ISP) for the resident based on needs and findings of the assessment.Graduate of an accredited school of Nursing (BSN) Current Registered Nurse New York State License. Minimum (2) two years of experience and/or training in providing services to older adults and/or residents; Related work experience may include but need not be limited to home care, residential care, adult care facilities, rehabilitation, health care management and the provision of nursing care in health care. PRI certification required or should be obtained within 6 months of hiring. At least 2 years of supervisory experience.Compensation: $105,000 min. to $110,000 max.
