Due to our extreme growth, we are looking for Bilingual (English/ Spanish) speaking HARP Care Manager to assist with completing assessments and care plans for patients who experience behavior health conditions and homelessness. An exciting opportunity for someone with an interest in working in healthcare and working with the underserved patient population. HARP Care Manager assist with assessing, managing care and addressing social determinants of health for Medicaid patients with chronic health conditions. HARP Care Manager Assessments and care plans are the driving force that connects patients with Health and Community Based Services (HCBS) / Community Oriented Recovery and Empowerment (CORE). HARP Care Managerserve as the lead contact with insurance companies and health and community-based providers.Build and maintain relationship with patientsConduct face to face assessments for all patients to assess their medical and social needsCreate a care plan in adherence with providers and caregiversProvide community resources to patients to ensure health and well beingPromote timely access to appropriate careIncrease utilization of preventative careSchedule appointments and transportationServe as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s) and community resourcesFacilitate patient access to appropriate medical and specialty providersEducate and refer patient to community resourcesKeep detailed up to date documentation2-years’ experience in social servicesBachelor's degree requiredBi-lingual Spanish strongly preferred$24.00-$26.00 an hourEssen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.
