USD $53,000.00/Yr.USD $60,000.00/Yr.S:US is seeking an LPN for our men's shelter in Brooklyn.This position is located at 1000 Blake Avenue, Brooklyn, NY 11208 The LPN meets with the Program Director, Director of Social Services, Registered Nurse, and Social Service staff to coordinate shelter residents to receive ongoing treatment in the community.In this position you will;Take vital signs, such as blood pressure, temperature and weightAssist with diabetes medication and treatmentTake medical histories and enter information in computer systemsCoordinate with Case Managers, Senior Case Managers and Director of Social Services to ensure proper care, nutrition and treatment plans are followedFollow up with community health care providers to ensure medical treatment plans are documented, monitored and followedHave flexibility to work a schedule that includes evenings and weekendsIND1#CBNew York State Licensed Practical NurseExperience working on a multi- disciplinary team and/or previous experience with homeless populationBilingual English/Spanish preferred but not requiredEffective August 30, 2021, unless granted a reasonable accommodation for disability, all new hires must show proof of vaccination against the COVID-19 virus.
