The Developmental Division strives to empower people with intellectual and developmental disabilities to live as they choose, actively engaged in the community, pursuing their goals and interests through a choice of supported residential options, health services, meaningful activities, and employment opportunities.Position Description: The Community Habilitation Specialist (CHS) will provide social skills training to children/adult with Developmental Disabilities as classified by the OPWDD. Sessions take place in the community or in the home depending on the need of the child/adult. a. The Community Habilitation Specialist displays a congenial, warm acceptance for all individuals.b. The Community Habilitation Specialist will primarily be responsible for the assessment and observation of the person's skills development.c. Is alert to the expressions of attitudes, feelings and relationships of each individual. Develops an awareness of the individual's needs, not only through the observation and interaction but through a study of the individual's case history and by appropriate consultation with other members of the staff.d. Support the person in navigating the program and identifying incentives in learning new skills and behaviors.e. Community Habilitation Specialist meet with the assigned children/adult for 1-2 times per week depending on the child/adult's needs. Community Habilitation Specialist will have 5 or more clients on their caseload.f. Community Habilitation Specialist will travel to various different locations throughout New York City.g. Community Habilitation Specialist attends supervision and training as required.h. It is essential that Community Habilitation Specialist know and protect individual's rights.i. Community Habilitation Specialists will create plans indicating goals the person would like to work on and the steps on how to achieve it.j. Community Habilitation Specialist will document each training session in the appropriate databases and report the progress and barriers with each client.k. The Community Habilitation Specialist will receive consultation and supervision from the Coordinator of Residential and Community Services.Please specify area of study:High School Diploma/GEDBSW or Bachelors in related field preferred. Bilingual a plus.Other Requirements: �?Familiarity of IRA regulations relative to residential habilitation�?Proficient in computer applications�?A clean and valid NYS driver's license required.�?Maintain all certifications.
