Find out how you can Make It Possible. Assesses the social and emotional needs of patients. Provides direct counseling/social services to students referred by self, health team, faculty, administration and other school ancillary staff. Provides group psychotherapyin clinic. Participates in outreach and engagement activities in the school community. Tracks and follows-up on all referrals made in and out of school. Coordinates social work services and patient referrals between the program and other agencies providing social and educational services in both the school and local community. Establishes referral network/linkages with outside agencies for both students and parents. Serves as an advocate for patients rights within the clinic and serves as a mediator between parents, child and school. Remains available for crisis intervention social work as the need arises or arranges for such coverage by emergency social services. Coordinates social work services on site. Consults with teachers/administrators on student problems in the classroom. Provides information on psycho-social issues related to class behavior, especially that relate to health concerns. Records professional assessments, identified problems, treatment plans and progress notes as required by the Department of Social Work Services and the School-Based Health Center Program. Acts as aliaison between clinic and school administration. Is competent to care for patients based upon the age groups treated. This is a Grant Funded 1199 position Preferred Criteria: At least 1 year of experiencein behavioral health crisis intervention Experienceproviding psychotherapy to children and adolescents Fluency in Spanish Required Criteria Experience or training in Hospital orSchool Based Clinics providing psychotherapy Works effectively with adolescents, families, professionals and other personnel from a wide range of cultural, social and economic backgrounds Knowledge of adolescent sexuality and health issues Masters Degree in Social Work (MSW) New York State Licensed Masters Social Worker (LMSW) Join a healthcare system where employee engagement is at an all-time high.
