Overview As the Director of Population Health Analytics within the Ambulatory Care Department of our large health system, you will lead a team responsible for analyzing clinical and operational data to support population health management initiatives. You will oversee the development and implementation of data-driven strategies to improve patient outcomes, enhance care coordination, and optimize resource allocation across ambulatory care settings. Responsibilities Establish and maintain positive relationships with patients, visitors, and other employees, interacting professionally, courteously, and appropriately. Provide strategic direction and vision for the Population Health Analytics Department, aligning objectives with organizational goals and priorities. Collaborate with senior leadership, clinical stakeholders, and external partners to develop and execute population health management strategies. Oversee the collection, aggregation, and analysis of clinical and operational data from ambulatory care sources, including electronic health records (EHR), claims data, and population health registries. Lead the development of standardized reports, dashboards, and analytics tools to monitor key performance indicators related to population health, care quality, utilization, and cost-effectiveness. Utilize advanced statistical methods and predictive modeling techniques to identify high-risk patient populations and forecast healthcare utilization patterns. Collaborate with care management teams to prioritize interventions, allocate resources, and tailor care plans for patients with complex needs or chronic conditions. Evaluate the effectiveness and impact of population health programs and interventions, assessing outcomes such as patient satisfaction and clinical outcomes. Generate insights and recommendations based on program evaluation findings. Qualifications Minimum of 5 years of experience in healthcare analytics, focusing on population health management and ambulatory care settings. Demonstrated leadership experience, including the ability to build and lead multidisciplinary teams. Expertise in data analysis, statistical modeling, and predictive analytics, with proficiency in tools such as SAS, R, Python, SQL, and healthcare analytics platforms.
