Care Transitions Nurse Manager - RN HealthCare Support is actively seeking a Care Transitions Nurse Manager - RN to fill an opening with a prestigious Healthcare companyin New York, NY. Daily Responsibilities for Care Transitions Nurse Manager - RN : Will be making field visits but would not have their own caseload. Assisting and overseeing some of the more complex cases - act as a SME for Community Health Nurses in the field Communicate with interdisciplinary teams, social works, dietitian, helping coordinate that care. Monday - Friday 8:30 am - 5:30 pm Required Qualifications for Care Transitions Nurse Manager - RN : Active NY Registered Nursing license 3+ years experience as a Registered Nurse / RN 2+ years of recent experience at a Certified home health agency (CHHA ) 1+ year of recent supervisory experience Benefits for Care Transitions Nurse Manager - RN : Medical, Dental, Vision HAS/FSA 28 days of PTO + 9 Paid Holidays Short/Long Term Disability Benefits Life Insurance, Wellness Programs, & Identity Theft Protection 403(b) ???????
