Conduct comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors) including mental status, and diagnosis.*Young Adult Assertive Community Treatment (YAACT) Team, Licensed Clinician**Reports: To Program Director*Travel or Special Conditions/Requirements: We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. In-person work primarily in Brooklyn and Staten Island.*JOB DESCRIPTION:*The YAACT team provides non-traditional community based mental health treatment to promote the recovery and community integration of 68 young adults with serious mental illnesses and co-occurring substance use disorders to reduce use of ERs and inpatient hospital stays. The licensed clinician delivers routine treatment services and has lead responsibility for integrating goals and services with the tasks of all team members and for providing psycho-education. The licensed clinician supports consumers in achieving wellness, recovery and positive relationships in the community. The licensed clinician will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights to self-determination, shared decision-making and collaboration. The clinician will utilize evidence-based practices and employ a trauma-informed and person-centered approach. * Obtain physical health and dental health history. * Assess use of substances or alcohol * Gather psychosocial information including education, employment, social development and functioning, and information on self-care, living situation, nutrition, money management and family. * Collaborate with families and social supports as indicated/requested by the participant for treatment needs. * Provide treatment services including problem-solving, wellness self-management, medication education, family support, relapse prevention, harm reduction and substance use counseling. * Provide 24-hour crisis intervention on-call services on rotating basis * Provide 80 percent of treatment contacts in the community * Document client progress to maintain a permanent record of client activity according to established methods and procedures. * Participate in daily morning team meetings and treatment planning review meetings. Follow the established YAACT policies & procedures, agency policies and maintain appropriate professional standards. * Use individual client data to inform clinical decision-making and identify areas for quality improvement; use data to track client progress over time. * Share client data with clients to help facilitate shared-decision making and keep client informed of their progress * Other related duties or projects as assigned..*ESSENTIAL & CORE FUNCTIONS: * * Experience understanding family systems based on diversity of cultural and ethnic backgrounds * Experience understanding various evidence-based models including but not limited to motivational interviewing, harm reduction, cognitive behavioral therapy and dialectical behavior therapy. * Extensive knowledge of Assertive Community Treatment, family community support services such as 12 step programs and other community resources * Proficient in computers * Highly organized and self-motivated * Familiarity with and ability to use an Electronic Health Record system. * Demonstrated commitment to diversity, equity, inclusion and cultural humility.*EXPERIENCE:* * The licensed clinician must possess and maintain an active license with a master's degree in the human services field such as social work or mental health counseling. LMSW or LMHC preferred. * The licensed clinician will have experience using culturally competent treatment approaches. * Knowledgeable about the psychosocial aspects of illness and health. * Ability to work efficiently in a fast paced environment. * Proficient with word processing, electronic health records, and electronic communications. * Well organized with strong time management skills.
