Fairview Health Services are looking for a driven In Clinic Visit Facilitator to join the hard working team at the Women's Health Clinic at our Riverside location on the University of Minnesota Campus. We believe the hours for this role will be 8:00-4:30. This role will be working with one of the most diverse patient populations in our M Health Fairview system. The providers with OB-GYN Riverside have an outstanding relationship with all staff, and we are passionate about education and supporting the team as a whole. Work-life balance is the best benefit of this position - no holidays, no weekends! You will also be asked to travel to our Highland Park and New Brighton sites on occasion. This Visit Facilitator is responsible for supporting in-person clinic visits, including communicating with patients prior to their visit and helping guide patients through the care experience. Visit facilitators capture pertinent medical information from the patient and detail the information into the medical record. This role also helps ensure the exam rooms are stocked, equipment and supplies are available, and assists with tasks that support the effectiveness and efficiency of the team. The Visit Facilitator may also assist in scheduling and coordinating appointments, preparing schedules, and other delegated duties as assigned. Prepare patients for their appointments including: communicating with patients in advance of their scheduled appointment to ensure medical records are collected, received, and available for the visit. Assist in the rooming process by collecting and detailing pertinent information: reason for visit, medication review, vitals collection with automated vitals machine, and health questionnaires. Support the patient connection with the provider for the visit and act as a resource while ensuring a positive patient/provider experience. Complete visit wrap-up/after-visit summary and ensure patients are aware of recommendations for follow-up. This may include assisting with scheduling and coordinating follow-up appointments. Communicate efficiently with patients, providers, and the care team, demonstrating emotional intelligence in person and with virtual technologies. Required At least 1 year of customer service experience. Proficient with technology (computer hardware and programs, smart devices such as phones or tablets). Preferred Current nursing, medical, social work, psychology, or pharmacy student. Successful completion of: NA, EMT, or HUC program. Experience with medical terminology, Epic Health Records, telephonic customer experience, and previously worked in a healthcare setting. Qualifications Compensation $20.01 - $28.25/Hr.
