To us, it's a privilege and our calling. ArchCare Senior Life (PACE) is a long-term Medicaid/Medicare plan designed specifically for seniors who need nursing home level care but want to stay at home. With Senior Life, patients have a dedicated Care Team to coordinate everything, so they are able to live comfortably where they are, knowing they'll get the coordinated care they need. Evaluates nutritional status of individual participants utilizing nutritional guidelines, professional knowledge, and interviewing skills to provide quality care. Develops and implements nutritional plans, monitors participant's progress and recommends/revises changes in nutritional plan for effective participant care. Documents changes in the participant's condition and details care provided by completing all required documentation and ensuring compliance to standards and policies. Coordinates with other members of the interdisciplinary team to ensure that the comprehensive plan of care is followed and revised as needed. Provides nutrition counseling to participants and/or caregivers utilizing educational materials, modified diets, and teaching aids to ensure participants understanding of the plan of care and its implications. Oversees the work of Geriatric Caregivers in the kitchen by working with Food Services Manager to recommend solutions regarding specific dietary needs.
