Job Description - Eligibility & Benefits Coordinator (250000FI) This position will require employees to be in the office four (4) days a week. The Social Security Information Manager is responsible for coordinating benefits under SSI/SSI Related eligibility categories and overseeing specific requirements for Medicaid, SSI, and SSI-related eligibility and benefits for CASSIP Enrollees. This individual shall establish and demonstrate collaborative relationships with SSA, ESA, and DHCF's Division of Managed Care; assessing and assisting HSCSN members with disabilities and their caregivers with the application process to acquire Social Security Income (SSI) and Social Security Income-related benefits. The Coordinator will conduct outreach to identify program participants needing assistance and will establish and maintain positive relationships with area healthcare providers, service organizations, and government agencies. Qualifications Minimum Education Bachelor's Degree in Public Health or another Human Services Related Field (Required) AndMaster's Degree in Social Work, Counseling, or another Human Service Related Field (Required) Minimum Work Experience 5 years Medical and/or behavioral health case management experience in a hospital, clinic, health plan or other health/behavioral health care services agency. (Required) And3 years Previous experience assisting D.C. residents with applying for federal and/or local entitlement benefits, social services, and/or human services. (Required) Required Skills/Knowledge Understanding of and experience with SSA disability regulations.Extensive knowledge of public benefits rules and regulations.Excellent verbal and written communication skills.Highly proficient with computer systems and applications. Computer literate in Microsoft suite.Strong organizational skills.Superior customer service skills. Ability to build rapport with individuals who may be experiencing challenging circumstances.High level of cultural sensitivity. Must possess knowledge, skills, attitudes and beliefs to work well with, respond effectively to, and be supportive of people in cross-cultural settings.Ability to define problems, collect data, interpret data and facts, and draw conclusions.Ability to make sound decisions and use good judgment.Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practice. Required Licenses and Certifications Licensed in the District of Columbia as a Licensed Independent Clinical Social Worker (LICSW) or RN, LGSW, LGPC, LSW. (Preferred)Driver's License (Required) Job Functions Essential job duties: Conduct initial member assessment for potential eligibility for disability SSI/SSI-related benefits. Assist individuals with disabilities and their caregivers via in-person meetings, electronic communication, and telephone to develop and file an electronic Social Security Administration (SSA) disability benefits application for Social Security Income (SSI) and Social Security Disability Insurance (SSDI) and monitor the status of submitted applications. Establish a list of case needs and complete online and hard copy documentation related to client benefit applications. Develop collaborative relationships with the SSA, medical providers, Care Managers, legal representatives, and individuals eligible for SSI/SSI-related benefits. Obtain and organize all pertinent medical evidence and draft reports as part of the SSA application for SSI/SSI-related benefits. Interview members for medical and functional summary reports. File appeal documents as needed and collaborate with SSA and Department of Disability Services on reconsideration of cases. Attend disability related appointments with providers and the SSA offices. Research and remain current on applicable rules and regulations with regard to SSI/SSI-related application and benefits. Provide outreach and basic need connections for members experiencing issues with housing and food and needing medical attention. Link members with appropriate community support systems during and following the SSI/SSI-related application process to ensure a broad continuum of care. Learn and interpret eligibility policies and procedures with regard to Medicaid-related benefits and services and perform screening activities. Accurately maintain all member records and documents and track all activity for each case. Assist with tracking clients for scheduled and ad hoc reporting. Adhere to all state and federal privacy regulations, including HIPAA, and to HSC policies and agreements regarding confidentiality, privacy and security. Conduct group activities designed to help navigate the disability application process. Conduct training activities and develop education materials focused on acquiring and maintaining SSI/SSI-related benefits. This job operates in an office environment. Physical Requirements Sedentary Work: Lifting 10 lbs. maximum and occasionally lifting and/or carrying such articles as dockets, ledgers and small tools. Although a sedentary job is defined as one that involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties. Travel Requirements Frequent travel requirements for this position. The incumbent should have a valid driver's license and access to a motor vehicle.
