Sign On Bonus Potential: Up to 15,000.00 Schedule - Shift - Hours: Full-time - Day and Evening shifts - 9:00am-5:30pm JOB SUMMARY Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of Carroll Hospital Center and Carroll Hospice. The Hospice Liaison serves in a consultant role for Carroll Hospice acting as an advocate for Carroll Hospice programs and providing education for healthcare professionals, patients, and families in the surrounding community. The purpose of this role is to improve care for all patients who have a life limiting illness and specifically to facilitate admission of eligible patients to hospice care in the setting of their choice. REQUIREMENTS Required Associate's Degree Graduate from an accredited school of nursing. Preferred Bachelor's Degree 2-4 years RN experience in hospice care, regulations, management, administration, marketing, education or discharge planning required. Ability to collaborate with others and to initiate and plan own work activities. Effective patient and staff teaching required. CPR - AHA Healthcare Provider RN - Registered Nurse SPECIFIC REQUIREMENTS Interpersonal communication skills (written or oral). Maintain effective contacts with a variety of hospital personnel and physicians; maintain documentation in a timely manner. Fleet Safety Requirements.
