Part Time Remote LCSW

Salvo health is looking for an experienced part time LCSW/LICSW to play an integral role on our care team. This role will work directly with patients, offering personalized psychosocial support and counseling to patients to reduce symptoms and improve quality of life. The LCSW will empower patients with the knowledge and tools to make lasting behavioral changes that will improve clinical outcomes and overall wellness. You'll serve as one of our patients' primary success partners, with responsibilities including: addressing member health questions, providing relevant educational resources, and fostering positive member accountability. If you have a passion for patient empowerment through a holistic-based approach to medicine, come join our team!Salvo believes that the most inclusive and equitable culture makes for a better business, so we welcome diverse candidates for this key role.ResponsibilitiesConduct comprehensive psychosocial assessments of patients with to identify needs, strengths, and challengesProvide exceptional counseling, disease management and health education to patientsDevelop and implement personalized care plans in collaboration with patients and an interdisciplinary healthcare team, focusing on behavior modification, coping strategies, and lifestyle changesSupport goal setting for individual patients synchronously and asynchronously to help them better manage their chronic conditions and associated mental health needsConduct regular outreach to patients, based on their needs and preferences, to support engagement and elicit behavior changeDocument patient interactions, progress, and care plans accurately and promptly in the electronic health record (EHR) systemCoordinate with clinical team and advocating for patients to provide an exceptional patient experienceDevelop and maintain professional, support-oriented working relationships with team members, including participation in case conferences, team meetings, and continuing education activitiesOffer crisis intervention and support for patients experiencing acute emotional or psychological distress; connecting with local resources, as requiredCreate and revise consumer-facing content advising on mental health best practices for chronic GI conditions and weight managementTranslate complex medical ideas into digestible messages for a lay audience, including performing research as required to ensure understandingWork with clinical team and outside advisors to draft policies and protocols to ensure consistency of LCSW care deliveryWork with a cross-functional product team to develop and constantly improve our in-app patient experienceRequirementsLCSW license with licensure to practice in NY or MI (also open to PA, FL, LA, OH)2+ years experience as a an LCSWBachelor's and Master's degrees required Experience communicating with patients asynchronously preferredExperience with GI conditions or weight management a plusProficiency in using telehealth platforms and electronic health record (EHR) systemsPassion for working with clients to facilitate behavior change by helping them set health goals to effectively manage chronic diseaseExperience working with patients in a clinic setting preferredDemonstrates a willingness to go above and beyond normal responsibilities to provide the best care possibleExcellent interpersonal communication and problem-solving skillsExceptional organizational skillsExperience and ability to thrive in a fast-paced, high-growth environmentStrong personal computing skills - remote work experience preferredPassion for and commitment to functional medicine approachesSalvo is a new approach to help millions of Americans facing chronic health conditions, centered on chronic gut health and metabolic conditions from IBS to obesity.
