POSITION SUMMARY The Float LPN is responsible for delivery of clinical care and works closely with medical, education and clinical staff to ensure the effective and efficient delivery of health care services to assigned students. SALARY $30.00 EDUCATION AND EXPERIENCE/QUALIFICATIONS Graduation from an accredited licensed practical nurse program required. Associates in a related field preferred. Must have 2 years of clinical experience providing nursing services to children and/or youth clients from diverse economic and cultural backgrounds, as well as exceptional communication, interpersonal and organizational skills are required. Contributes to the creation of a compassionate and caring environment for students and families through display of kindness and active listening. Provides direct care to students, carrying out orders given by the medical provider, including obtaining student medical histories, obtaining vital signs, preparing students for and performing diagnostic testing, assessing and providing wound care, dressings, and administering injections. Accompanies assigned students with medical needs on off ground activities and job sites as required, as well as within the school building. Demonstrates effective teamwork by interacting in a positive manner with colleagues, visitors, students, caregivers and families. Contributes to a collaborative work environment by initiating open communication, a positive intent and offering assistance to support students, families and care givers. Contributes to a safe and secure work environment for students, visitors and colleagues by following established procedures and protocols. Respects student and family cultural, spiritual and personal norms by acknowledging the essence of each person and appreciating and responding to the uniqueness of each person. Provides chronic care disease support to the students in all aspects of the care, treatment and services provided and in accordance with the clinic's integrated model of care. Develops student focused self-management goals and care plans with the student. Ensures gaps in care for screenings, chronic care tests, (such as HgbA1c for Diabetics), health literacy assessments, tobacco screenings, BMI, etc. are completed in timelines established by best practice. Attends and provides input for students in multi-disciplinary team meetings, consult meetings, PPT meetings, and other meetings as necessary. Demonstrates strong nursing knowledge base and skill in job performance. Assumes responsibility and accountability for coordinating care, in collaboration with medical, educational and clinical providers. Ensures that continuity of care is maintained in all aspects of student care. Contributes to medical staff's effectiveness by identifying student care issues that must be addressed; providing information; recommending options and courses of action; and implementing medical providers' directives. Serves as liaison to medical, educational and clinical providers. Protects students and employees by adhering to infection-control policies and protocols. Documents timely and accurately student care services in the student record. Tracks referrals, labs and diagnostics in accordance with policy. Must be able to pass a respirator fit test at time of hire and on an annual basis. Trains and participates in emergency safety interventions authorized and approved by the agency, when a student presents with an imminent safety concern. Participates with regular staff meetings focused on coordinating care within an interdisciplinary team. Performs job responsibilities consistent with standards for best practice nursing, including excellent communication with all providers to ensure the delivery of high quality care. Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture. Regulatory and Corporate Compliance Responsibilities Understands and adheres to Wheeler Clinic's compliance standards as they appear in the clinic's Corporate Compliance Plan, Code of Conduct, and Conflict of Interest Policy. Completes all training in a timely manner as required by state and federal regulations (e.g., Compliance, Fire Safety, HIPAA HI-TECH, Infection Control, Sexual Harassment, etc) and specific to the department and position. Keeps abreast of all pertinent federal, state and clinic regulations, laws, and policies as they presently exist and as they change or are modified. Performs other related duties as assigned.
