Terros Health is hiring a Licensed Practical Nurse for our Desert Cove location in Phoenix, AZ. Full-time; 40 hours/week Monday-Thursday, 7am-5:30pm Compensation: $26-$29/hour $2K Starting Bonus* * New Hires Only Location: Desert Cove Clinic; Near I-17/ Peoria Rd The LPN performs duties in support of therapeutic services and provides comprehensive direct support services which are structured to meet the needs of an adult population composed of SMI, dual-diagnosed and/or referred persons. Duties include, but not limited to: * Provides nursing services to members including engagement, assessment, treatment planning, medication administration, education and supportive psychotherapy * Documents all activities in the clinical record including member's responses to medication administered and according to policy using approved forms * Coordinates physical and behavioral health care needs for members * Prepares and administers oral, intramuscular, and subcutaneous medications; performs TB testing and venipunctures * Identifies potential barriers to quality care and opportunities for improvement and reports them to the Lead Nurse Full Benefits Package including, but not limited to: Medical Insurance - BCBS - Multiple Plans (PPO and HDHP) Spending Accounts (HSA, FSA, LPFSA, DCFSA) Critical Illness Insurance and Hospital Indemnity Dental & Orthodontia and Vision Plans Voluntary Life/ AD&D and Short and Long Term Disability Insurance Identity Theft, Prepaid Legal Pet Insurance 401K Employee Assistance Program GCU Tuition Discount for Employees and Dependents Generous PTO: 4+ Weeks + Holidays (PTO Accrued from 1st Day of Employment) + 1 Floating Holiday Qualifications * Completion of the applicable Licensed Practical Nurse course of study and degree * Current licensure issued by the State of Arizona Board of Nursing as a Licensed Practical Nurse * Minimum of one year relevant experience in a behavioral health setting preferred * Knowledge of DSM-V and SMI diagnoses, psychopharmacology, substance abuse, additions, recovery, relapse prevention, and 12-step treatment philosophy. Knowledge of the principles of cultural competency and adherence to it's philosophy. * Must have a valid Arizona Fingerprint Clearance card or ability to apply for an Arizona Fingerprint Clearance card (Level 1) within 7 working days of assuming role * Must have valid Arizona driver's license, be 21 years of age with minimum 3 years driving experience, and meet requirements of Terros Health's driving policy * Must pass a drug screen and TB Test * Background Check required * Physical Demands: Must have high manual dexterity and tactile sensation in hands and fingers to allow for completion of medical procedures. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception. * Minimum typing speed of 25 wpm
