The Multnomah County Health Department's Primary Patient Care team is looking for a Licensed Practical Nurse who deeply desires to make a difference in their community. This position provides focused community and clinic health services towards the prevention of disease, the promotion of health, the reduction of disability, improving the client's self-care, using professional judgment in focused/limited assessments, screening, contributing to care planning, intervention and provides health information/education to patients. We're seeking a visionary and dynamic leader who is creative in problem solving and contributes new ideas and solutions to the organization to join our team in Integrated Clinical Services The Licensed Practical Nurse works within the health care team and provides focused community and clinic health services directed towards the prevention of disease, the promotion of health, the reduction of disability, improving the clients self-care, using professional judgment in focused/limited assessments, screening, contributing to care planning, intervention, and provides health information/education to patients. The Panel Manager role tracks the health of the population of patient's assigned to the provider panel. Manages team communication including phone messages, fax requests, and warm hand-offs, as needed. The Panel Manager is responsible for the team InBasket within the electronic health record system and delegates requests to appropriate staff, in addition to completing those requests that are within the scope of a LPN. The Panel Manager completes needed patient visits as noted on EPIC team schedule in addition to providing focused assessments to walk-in clients. Supports providers when staffing indicates the need. As a Licensed Practical Nurse, you will provide : Panel Management * Utilizes electronic reports and review of provider schedules to proactively assess needs for prevention screening and communicates via EHR. * Pro-actively performs pre-visit activities (e.g. scrubbing and huddling) for client visits, as needed. * Works with Providers and team to monitor chronic disease and prevention reports and initiates follow-up with clients or ensures appropriate referrals as needed. * Forecasts immunizations. * Collaborates with Team Clerical Assistant to ensure appropriate tracking and follow-up of referrals. * Leads the team and directs staff to complete duties, within scope, to support smooth clinic flow. * Services are provided in Spanish and English. Team/Client Communication * Manages team cell phone, desk phone, communicates normal lab results and messages from the care team. * Manages Provider and team electronic and paper in-baskets. * Schedules appointments as needed. * Follows-up on specialty referral reports, clinical treatments, prescriptions, medication cancellations, etc. * Participates in patient reminder calls and follow-up to no-shows, as indicated. * Documents clearly in patient charts, using the SOAP method, if indicated, and EPIC "dot-phrases" and "smart phrases" as deemed appropriate per patient encounter. * Communicates with all staff professionally and effectively. * Services are provided in Spanish and English. Provider/Team Support * Supports back office clinic flow by direct provider support as needed including rooming patients, ordering labs as directed, giving immunizations and medications (injections, inhalants, rectal, oral). * Assists clinicians with procedures. Provides after visit instructions based on nursing experience and understanding of client needs; makes follow-up appointments, if necessary, and communicates referrals. * Obtains vital signs and appropriate screening; collects clinical/nursing assessment data from patients including focused assessment, of on-going health issues (e.g. depression, diabetes, asthma, wound and pain management, and other chronic diseases). * Prepares patients for exams, and procedures, including equipment set-up and specimen collection. * Administers medical treatments such as cleaning and dressing wounds, ear irrigations, suture removal, catheterizations, fluoride treatments, etc. * Performs diagnostic tests such as labs, phlebotomy, EKG, etc. Completes Nurse standing orders per assignment. Provides patient education. * Assists clients with completion of forms (e.g. Head Start). Communicates referrals to Team Clerical Assistant if not done. Maintains clinical skills as dictated by the client population and the clinical practice (adult, pediatric, OB, internal medicine). * Reviews cases with the providers and team as needed. Works with RN and PCP to contribute to the plan of care. Follows established plan of care. * Seeks expert advice on issues for clients and provides input to providers and nurses on client response. * Completes competencies required for the position. Completes documentation and charting as required by policy. * Services are provided in Spanish and English. Resource Management and Quality Assurance/Improvement * Participates in equipment/supply checks. * Troubleshoots equipment issues and reports necessary repairs, as needed, to supervisor. Follows downtime priority and project lists. * Provides feedback for completed tasks, as needed for communication flow. * Provides back-up support to other team members as needed and other duties as assigned. * Completes assigned Quality Control/Assurance activities. * Actively participates in process improvement, when assigned. * May perform Vaccine Lead duties, if assigned. * Services are provided in Spanish and English.
