Full-Time Licensed Practical Nurse (LPN)

American Medical Associates - Licensed Practical Nurse (LPN) Needed - South Yonkers- Assisted Living  **Salary- up to $38.00/hr (based on expereince)**   Qualifications: • Graduate of an approved school of Licensed Professional Nursing • Currently licensed as a Licensed Professional Nurse in the state of New York • Assisted living experience preferred   • Good verbal and written communication skills Responsibilities: Calls the home care coordinator, RN or supervisor with verbal report of initial findings. In conjunction with the family and physician and/or authorized practitioner and Americare Supervisor adheres to   the Plan of Care based on the RN's assessment. Provides nursing care as outlined in the Plan of Treatment to include the following: Nursing services, treatments and preventive procedures including those requiring substantial specialized skill and ordered by the physician or authorized practitioner. Initiates preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety. Evaluates the effectiveness of his/her nursing service to the individual and family. Observes signs and symptoms and changes in the patient's physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary.     #6617
