You are a key member of our clinical teams, setting an example of compassionate, professional care for others to follow.Summary: Vincentian Schenley Gardens, a senior community in Pittsburgh's Oakland, is in need of a Licensed Practical Nurse to join our Clinical Excellence Team. According to specific assignment and in accordance with established standards, the Licensed Practical Nurse provides technical nursing care, assists with resident care planning and observation of resident's condition, and reinforces physician, resident and family instructions. It is the duty of all employees to promote and support a resident centered care environment that ensures the treatment of all residents, family members, visitors, fellow employees and customers with kindness, respect, and dignity. Essential Duties And Responsibilities:Commits to the Vincentian Collaborative System (VCS) mission to nurture and sustain a ministry of compassionate care that preserves the human dignity of persons within a diverse and changing society.Upholds and promotes the VCS values of spirituality, compassion, quality, dignity, collaboration, advocacy and stewardship.Promotes and supports a resident centered care environment that ensures the treatment of all residents, family members, visitors, fellow employees and customers with kindness, respect, and dignity.Adheres to all facility and department work rules, policies and procedures.Performs individualized personal care for residents in accordance with the established nursing care plan and within scope of licensure.Collects electronic and paper data, reports and records observation of residents' condition.Participates in resident care conferences.Performs service duties and specific assignments such as cleaning instruments and equipment, collecting specimens, cleaning work areas and replacing supplies.Attends required continuing education programs and unit staff meetings.Assists in the orientation of new personnel.Maintains confidentiality of resident and staff information.Anticipates and takes action to maintain a safe environment. Incorporates safety/risk management standards into daily work practicesAssumes responsibility, in the absence of Registered Nurse or when directed by Registered Nurse, for duties such as:Administering injections and treatmentsMaking rounds with physician and assisting in examining and treating residents. Notifies physician of changes in resident's condition and any unusual or abnormal observationsAssists in developing nursing care plansAssists with admitting, discharging, transporting and transferring residents.Reviews physician's orders, transfer and pre-admission information for additions, omissions, discrepancies, etc.Reviews the resident's medical and social historyRequisitions and arranges for diagnostic and therapeutic services as ordered by resident's physician within the facility or outside facilitiesOrders prescribed medications, requisitions supplies and equipmentNotifies dietary department of new orders and changes in resident dietsAssembles residents charts, assures all permits, authorizations and releases and other required forms are attachedTranscribes physician's orders to resident's medication and treatment record, etc.Maintains electronic health records and paper records.Assists with supervision and care assignments of nursing personnelNotifies physician of changes in resident's condition and any unusual or abnormal observationsPerforms all other duties assigned by supervisor.Required to support the organization's Code of Ethical Conduct and Compliance Program by timely reporting any actual or suspected compliance, fraud and abuse, ethics or policy violations.Education/Experience:1. Current licensure to practice practical nursing in the Commonwealth of Pennsylvania in good standing or a Temporary Practice Permit required.2. Appropriate clinical knowledge of nursing care, significant signs and symptoms of disease, sterile and aseptic techniques, nursing procedures and treatment, and developed skills to provide technical nursing care required.3. Current CPR Certification preferred.4. Previous experience as a Licensed Practical Nurse in long term care preferred.5. Previous experience with electronic health records preferred.Work Environment: Employees of a long term care facility work in a resident care environment that operates 24 hours a day, 7 days a week, 365 days per year. Employees can be expected to be exposed to unpleasant odors and may encounter physically or verbally aggressive residents.Working conditions include normal indoor temperatures. There may also be times when employees perform duties outdoors. Depending on the season, outdoor temperatures will vary. Employees may come into contact with residents who have infectious diseases or to resident bodily fluids or blood.
