Job Type Full-timeDescriptionDescription:Primary Job Function: Provide care, treatment and support to patients and assist in providing an environment of healing. Administer medications PRN, administer TB tests, attend patient provider meetings, listen to patients and document on patients medical and emotional status. Ensure the environment is safe and patients feel secure.Essential Job Functions:Observe patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action.Administer hypodermic injections PRN, and noting times and amounts on patient's charts.Administer and read TB tests, and document appropriately and timely.Assess and evaluate all patient-related medical concerns.Develop care plan, and follow up regarding all recommendations made by staff and coordinate care with community providers.Work as part of a multi-disciplinary team to assess patient needs, plan and modify care, and implement interventions.Evaluate nursing intervention outcomes, conferring with other multi-disciplinary team members as necessary.Collect samples for testing.Sterilize equipment and supplies, using germicides or sterilizer.Make appointments, keep records, or perform other clerical duties.Monitor patients' physical and emotional well-being and report unusual behavior or physical ailments to Supervisor and Program staff.Observing and documenting patient appointments with provider team.Assessing patients' health conditions, obtaining medical histories, and reviewing patients' health records.Educating patients on the importance of med compliance.Helping with appointment scheduling and coordination with medical providers.Thorough communication with clinical and administrative staff regarding patient health concerns.Ongoing collection of patient information through assessments and logging vitals, height, weight, temperature, blood pressure, pulse, and respiration.Collaborating with medical providers prior to patient appointments to communicate clinical teams concerns.Evaluating patient wellbeing, reporting any needs to medical providers.Collaboration with case managers and outpatient director with patient concerns and/or issues with med compliance.Assisting medical providers with preapprovals and other documentation.Document to The Joint Commission, DSS and DHCS standards.Additional tasks as assigned.RequirementsRequirementsMust have a valid California license as a LVNMust have a Valid driver's licenseExperience working in mental health, alcohol or drug abuse, or eating disorders a plus.Salary Description $25 - $35
