Our residents share a bond of mutual respect with our staff that builds memorable and lasting relationships.OverviewUnder the direction of the Director of Nursing, the Licensed Practical Nurse (LPN) is responsible for providing quality care to residents of the assigned neighborhood. The LPN functions as a professional caregiver who is accountable for planning, implementing and evaluating all aspects of resident care, including physical, psychosocial and environmental needs. Essential FunctionsDocuments according to nursing practice standards.Administers medications accurately and in a timely fashion and provides treatments to residents according to physician orders.Provides physical comfort and a safe environment.Accurately carries out physician/provider orders in a timely fashion.Initiates, implements, and evaluates care plans/MDS on an ongoing basis.Accurately completes admission and discharge procedures.Coordinates staff and resident care appropriately to meet resident needs.Follows all policies and procedures, annual mandatory in-services, required meetings, and consistently meets other conditions of employment.Performs other duties as assigned.Education and ExperienceCurrent CT LPN license is required.2 years of full-time clinical experience in nursing is required.Must be able to speak, read, and write the English language in an understandable manner.Comprehensive knowledge of geriatric nursing care, principles of nursing, and physical evaluation skills is preferred.Basic knowledge of human relations skills necessary to deal effectively with the geriatric residents, their families, and CHHC staff.Hours3rd shift: 11:00pm-7:30am, 32 hours per weekPay$30.00 per hour + shift differential (Weekday Nights: $2.00 p/h, Weekend Nights: $3.00 p/h)What are the benefits?Working in a Life Plan community will bring out the best in you.
