Klickitat Valley Health is excited to announce a unique employment opportunity for a kind and caring ARNP who feels called to serve some of our patients in greatest need. This position is a blend of Primary Care and Palliative Care and caters to patients with serious, progressive illnesses and their families. Primary Care providers see 13-17 patients / day. Palliative Care currently manages a census of 50 patients and are looking for a provider to help expand this great program. 4 day work week, no call, no OB, excellent benefits and competitive salary. $10,000 signing bonus and relocation bonus available. KVH is an eligible site for state and federal rural loan repayment.Applying for this role is straight forward Scroll down and click on Apply to be considered for this position.The Family Medicine team consists of 3 physicians and 6 mid-level providers. The clinic is a designated Rural Health Clinic (RHC) and also offers ExpressCare, Pain Management, Behavioral Health, Optometry, and Dental. Each FM provider has dedicated support staff and on site access to EMR support. The EMR is Meditech Expanse. The leadership structure in the clinic consists of a Medical Director (Physician), Clinic Director, Medical Support Staff Supervisor (RN) and Office Support Staff Supervisor.Our compassionate Palliative Care team is composed of nurses, social workers, chaplains and volunteers and works in tandem with our Hospice team. Most patient visits will be conducted in the clinic office setting while some are conducted in patient homes as well as virtually. The Palliative Care Provider will conduct chronic and complex chronic care management, advanced care planning and transitional care planning to support patients in achieving their goals while living with chronic disease.Benefits include medical/vision/Rx, HSA/FSA, dental, long term disability, retirement plan, medical services credit and a generous amount of Paid Time Off.Minimum Education, Training & Experience (includes licenses or certifications):Education / Licensure:Graduate of an accredited school of nursing with an active/current RN license in Washington State.Masters or Doctoral Degree from an accredited school recognized by the American Association of Nurse Practitioners.Current unrestricted registration/license in Washington State as an Advanced Practice Nurse.Certification as a Nurse Practitioner from the American Nurses Credentialing Center or American Academy of Nursing preferred.Current BLS and ACLS/PALS certifications required.Experience:At least one (1) year recent full-time experience providing direct patient care in an outpatient or acute care medical setting is preferred.ESSENTIAL FUNCTIONS / RESPONSIBILITIES (but not limited to):Reviews data/information such as: vital signs, height and weight measurements, etc. obtained from the patient as part of the clinic visit.Obtains patients health history (history and physical); performs various patient examinations/ physical assessments such as: pap tests, breast exams, general physicals, wellness exams, etc. (including biological, social and psychological needs); uses the principles of growth and development to assess each patients age-specific needs and provides age-specific treatment and care.Diagnoses, treats and plans appropriate health care plan for patients (based on scope of practice) and establishes goals/objectives related to the plan of care based on the unique needs of the patient; assesses patients response to treatment and care; consults with and/or refers patients to other physicians/providers and/or outside agencies/facilities as required.Diagnoses and treats acute/episodic minor illnesses (respiratory illnesses, ear and throat, gastrointestinal, genital urinary infections, dermatology, etc.) and other injuries.Screens for the presence of chronic illnesses and monitors the ongoing treatment and therapy of patients with chronic stable illnesses by providing pharmacological and counseling interventions.Performs a variety of procedures in the care of the patient, including but not limited to: suturing, cauterizing, skin biopsy, liquid nitrogen treatment, etc.Determines the need for, orders and interprets x-rays, CT scans, MRIs, EKGs/ECGs, mammograms, diagnostic ultrasounds, etc. as needed.Develops, initiates and maintains a preventative health monitoring program (cholesterol tests, blood pressure tests, diabetic screens, etc.) to assist patients with wellness care.Follows KVH procedures regarding the provision of patient care and pletes patient program and billing records in a timely manner and in accordance with all applicable standards, regulations, or requirements that may be set forth by contract. Orders/performs laboratory procedures and interprets lab test results for the patient.Accurately records patient information and clinical data (chief complaint, symptoms, medical condition, history, allergies, immunizations, etc.) in the electronic medical record (EMR) to maintain the paperless charting system of the clinic.Prescribes and/or provides medications, injections/vaccines, etc.as needed within scope of practice.Provides patient education that facilitates recovery, self-care, health maintenance, health promotion and wellness; documents patient teaching, tools utilized and patient understanding.Performs other duties as assigned.Job Type: Full-timePay: $113,400.39 - $161,681.84 per year
