Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency.Are you the right applicant for this opportunity Find out by reading through the role overview below.We are seeking a qualified *Licensed Practical Nurse (LPN) *interested in joining our ICM/ACT team at our Wilmington, Delaware location.*This is some information about the opportunity we're offering, and you may be searching for:* Monday-Friday, day shift, 40 hrs. workweek, office environment and community-based work, with company vehicle provided for this, business casual to casual attire and more.The *Licensed Practical Nurse (LPN*) as part of the ICM/ACT multidisciplinary team provides the coordination of both the behavioral and physical health care needs of the program participants, monitors participants medication and provides psychiatric nursing services consistent with physician's orders.
