New competitive salaries, immediate openings available, generous time off packages, DailyPay, 401k + match and so much more!If the following job requirements and experience match your skills, please ensure you apply promptly.We offer:Great culture and team atmosphereComprehensive benefits effective the first of the month401(k) retirement plan with a generous company matchGenerous time off accrualsPaid holidaysMileage reimbursementTuition ReimbursementEmployee Referral ProgramYearly Merit IncreasesEmployee Discount ProgramsWork/life balanceRN What You'll Do:New competitive salaries starting at $95,000/year.Flexible scheduling.Starting Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process.Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary itiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care.Provide regular visits to re-assess the patient's needs and update the plan of care.Perform clinical pronouncement of death and provide support to the family.Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely.Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines.LPN What You'll Do:New hourly rates.Flexible start/stop times.Provides direct patient care as defined in State Nurse Practice ActImplements current nursing practice following a comprehensive assessment and the plan of care.Provide accurate and timely documentation of patient services to reflect the plan of care.Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.Participates in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, MSW, Dietitian or Clinical SupervisorProvides appropriate pain/symptom management.Evaluate the patient's response to treatments/medications.Qualifications:Registered NurseGraduate from an accredited registered nursing program.Licensed as a registered nurse in the state of practice.LPNGraduate of an accredited practical nurse or vocational nursing programLicensed as a licensed practical nurse in the state of practice.Prior home health or hospice experience preferred.Must possess current CPR, First Aid Certification.Must be able to pass a background check and drug screen.Valid driver's license and proof of insurance is required.#ACHOS
